Your First Visit (What to bring with you):
- A list of the supplements and medications you are taking
- blood work/imaging studies done within the last 6 months (if you do not have a copy, we can request it from your medical physician at the first visit)
- Completed intake forms. Our receptionist will send you these forms.
Please Note: There is a 24-hour cancellation policy in effect. You will be charged 50% of the visit fee if you do not cancel before 24 hours. If you simply do not show up for your appointment (without notice), 100% of the visit fee will be charged. Your credit card information will be recorded at the first visit.
INSURANCE COVERAGE:
Naturopathic medicine is not covered under OHIP, however, it is covered under most extended healthcare plans / third party insurance. The invoice presented to you at the end of each session is used to submit to your insurance provider to receive reimbursement.
HST:
Naturopathic medical services are exempt from HST in Ontario. There may be HST charges on lab tests and products.
